On August 24, 2023, The Center for Medicare and Medicaid Services (CMS) announced that the Medicare Shared Savings Program (MSSP) saves Medicare more than $1.8 Billion in 2022, which is significantly more savings than in 2021 ($1.66 billion in 2021). Approximately 63% of participating ACOs earned payments for their performance in 2022. It appears that low-revenue ACOs outperformed high-revenue ACOs: 

  1. Low Revenue ACOs had a net savings of $228 per capita 
  2. High Revenue ACOs had a net savings of $140 per capita
  3. ACOs made up of 75% primary care clinicians saw the highest per capita net savings of $2941

Low-revenue ACOs are primarily comprised of physicians and may include a small hospital or serve rural areas.

While savings is a major component of the program, it is important to note that the program has high quality scores. In fact, all ACOs have achieved a quality score of 70% or higher, with an average score of 81.53%.  

For the 2023 performance year, the MSSP program has over 573,000 participating clinicians and approximately 11 million Medicare beneficiaries. Given the increased success of the program, CMS has set a goal that 100 percent of people with Traditional Medicare will be part of an accountable care relationship by 2030. Let’s hope the value-based programs (MSSP and others) continues to grow to provide increased value to patients: improve quality of care and lower costs.
